The Davis Vision Discount Program is under the Health Select plan. When using the Davis Vision Discount Program, HealthSelect (HS) participants should continue to follow network guidelines for your annual routine eye exam by using a HS network optometrist or ophthalmologist. However, the Davis Vision program can be used when purchasing frames, lenses, and contacts.
This benefit is available to employees and their covered eligible dependents that have elected the health insurance coverage. The covered member and their covered eligible dependents will be able to save money at Davis Vision Discount Program network locations. Please see the information below for a summary of benefits.
Please keep in mind that EyeMed Vision Care is an alternative vision program that is not covered under the Health Select plan. Davis Vision Discount Program and EyeMed Vision Care are independent plans and the benefits do not run concurrently.

Blue Cross and Blue Shield of Texas* (BCBSTX) is very pleased to provide you with this information about your value-added vision program through Davis Vision, Inc., a leading national provider of routine vision care programs.
Who are the network providers?
Participating providers include a wide selection of private practice doctors and the convenience of retailers such as EyeMasters and Wal-Mart.
Please call Davis Vision at 1-800-501-1459 to obtain a list of the network providers nearest you, or you may access our Web site at www.davisvision.com and utilize our “Open Enrollment” feature. You will be prompted to enter your Client Control Number, which is 4495.

How do I receive services from a Davis Vision network provider?
For providers other than EyeMasters or Wal-Mart, call the network provider of your choice and schedule an appointment. Many locations will accept outside prescriptions; please confirm this service when making your appointment.
Identify yourself as a Davis Vision plan participant and a BCBSTX (Plan 400) member.
Present your BCBSTX ID card at the time of your visit.
Your Davis Vision ID number will be the same as your member number on your BCBSTX ID card. It’s that easy to receive services!
