Effective September 1, 2003, each participant must pay a $50 deductible before co-payments are charged. |
Participating
Pharmacies: |
$10 generic, $25 preferred brand name, $40 non-preferred brand name, up to a 30-day supply. If available, the generic drug is mandatory or you must pay the difference of the cost between the generic and the cost of the brand name, plus the generic copay. You may fill your long-term initial prescription and one refill at a participating retail pharmacy. All additional refills of long-term medications must be filled through mail order.** |

Pharmacies: |
Up to a 30-day supply; you will be reimbursed 60% of the lesser of the amount you pay for the prescription minus you copayment. Please note that if a brand name medication was dispensed that has a generic available, your are also responsible for the difference between the generic and preferred or non-preferred brand name. |

New limits apply to refilling prescriptions at participating retail pharmacies. Effective May 1, 2003, HealthSelect participants can get an initial prescription and one refill at a participating pharmacy. After that, long-term medications (up to a 90-day supply) must be purchased through mail order at 30 generic, $75 brand name preferred or $120 brand name non-preferred. |

Effective July 1, new "retail maintenance co-payments" will allow participants to purchase their maintenance medications at local pharmacies. The retail maintenance co-payments will be $15 for generic drugs, $35 for preferred brand name and $55 for non-preferred brand name for up to a 30-day supply. |
