ICS 1 FORM - Use to add or revise a program or award.
| New or revised PROGRAM and AWARD information: Examples include addition of a new program; revision, deactivation, or closure of an award; changes to degree outlines or course sequence in existing programs; changes in assessment and test scores for placement; changes or additions to Prior Learning Assessment opportunities.
Note: For new programs, an ICS2 form must also be submitted for each new course. A degree outline should also be submitted for new programs or changes in course sequence. |
ICS 2 FORM - Use to add or change an existing course. | New or revised COURSE information: Examples include adding a new course, deactivating a course, changing an existing course including course number or rubric, course title, course description, prerequisites, corequisites, learning outcomes, lecture/lab/contact hours, approval code, type of instruction, etc. Note: When revising a course, only complete those sections of the form that apply to the revision. Including information in other sections of the form will lead reviewers to believe that other changes are being made and could delay approval. |
ICS 4 FORM - | Learning Outcomes and List of Competencies Worksheet (SCANS). Submit along with ICS 2 form when adding a new course or when revising learning outcomes. |
ICS 5 FORM - | Summary of Outcomes and SCANS Competencies for a Program or Discipline Area. Submit with a new award. |
Special Topics Form - Use to add a WECM Special Topics course. | Special Topics courses should be used only when course content does not exist in a current WECM course. These courses are provided for temporary use or transitional content. Special Topics courses are identified by a "9" in the third digit. |
Local Need Form - Use to add a technical course that is not in WECM. | A local need course may be used to respond to unique local or regional conditions or to regulatory changes. Evidence of need for this course must be substantiated by support from business and industry in a specified geographical area. A Local Need course should be submitted only if the course will be offered consistently over a period of time, rather than intermittently as with a Special Topics Course. WECM Local Need courses are identified with a "7" in the third digit of the course number. |
Out-of-State/Out-of-Country Credit Course Request Use for Study Abroad Proposals. | This form is part of the Study Abroad proposal submission. Additional required documents include an itinerary and course syllabus. Please consult the Faculty Study Abroad Handbook for deadlines and program guidelines. |
Unique Need Course Form Use for approval for a course not available in the ACGM. | Approval for a course not available under an ACGM approval number or for one with credit and/or contact hours in excess of the limits prescribed by the ACGM must be approved by the Coordinating Board. To be approved the course must be acceptable for transfer to two or more Texas and/or regional universities. Copies of letters documenting transferability must be included in the application. See the ACGM for additional guidelines. |
